Email Functionalmedicinepharmacist@gmail.com with your name, best email , phone number, and reason for consultation.

Step 2 Intake & Payment

You will receive a welcome email from our team with directions to filling out your intake form and waiver.  You will fill out medical history, medication list, allergies, family history and your treatment goals and return email the forms. You will also receive the link to pre-pay your $200 consultation fee. 

Step 3 Start Your Regimen

Once your consultation fee has been paid , your functional medicine pharmacist will carefully review your full history and intended goals to determine the best suited regimen. You will receive an emailed prescription for your personalized nutraceutical regimen.  Simply create an account for the online dispensary and order your supplements and they will be delivered straight to your door. Supplements are an additional cost.   I highly recommend automatic monthly refills so that you don’t have a lapse in your treatment as it is critical that you are compliant with your regimen in order to be successful . Contact your functional medicine pharmacist via email with any questions or concerns to ensure a successful initiation of your regimen.

Follow up consultations are recommended every 6 months. Email your functional medicine pharmacist to schedule your follow up  evaluation.
